It's Easy to Send a Smile! 
1. Download and complete the Send a Smile order form by clicking here.

2. Once you are happy with your order, save the PDF to your desktop and return to this page to submit your order.  

​3. Submit your order by clicking on Choose File (below) and upload the saved PDF from your desktop.

4. Once the file uploads, click the submit button. A confirmation message will appear when your order is successfully submitted
